Motion to Move Forward with Development of Comprehensive Health Plan

Recommended Action

I move that the Board approve action to move forward with developing a comprehensive health plan that focuses on the 10 vital services areas for a health community. Subcommittee can be created for a more focused effort.

Body

The Health Council shall develop recommendations to County regarding comprehensive health planning, coordination of health resources and identification of needs, services, projects, and programs that can affect community health.

 

The Health Council shall prepare Community Health Plan (5-7 year time span) periodically and report its Plan to County Council and to the State of New Mexico Department of Health. The Community Health Plan shall include:

 

(1)                     a county health assessment and inventory of health resources.

(2)                     identification of health priorities determined independently through the health council and a community based planning process; and

(3)                     strategies and resources to address health priorities.

 

The Health Council shall monitor health and health care programs and services in order to identify potential gaps and to reduce potential duplication;

collaborate with other entities to develop programs, networks, partnerships and coalitions as necessary to improve health; advise County Council

regarding policies that affect health; facilitate communication among local jurisdictions, state agencies and other entities; and identify additional public

and private resources to improve health in its respective jurisdiction.

 

The Health Council shall review the health plan every year to ensure the Health Plan developed is still meets the needs within the community. If there are

any changes or edits, the Health Council will present changes to County Council.
